Legally, a person in Bhutan is considered a child until their 18th birthday. Depending on the context, adolescents are recorded as people aged 10 to 19 or 15 to 24; the health sector usually defines young people as those aged 10 to 24. Bhutan's national programme for elderly care (National Elderly Care Programme) uses an operational age threshold of 65 and older, while the term senior citizen has no uniform age limit. For children, early childhood care and development, schools, health services, counselling and protection services work together. The Integrated Child Care and Protection Programme supports, among other things, parenting, nutrition, mental health, protection and digital safety. The National Commission for Women and Children (NCWC) handles complaints, child protection cases and case management. Schools, health centres, voluntary health workers, community programmes and civil society organisations take on additional responsibilities depending on the case. According to UNICEF data, birth registration among children under five was 100 percent. At the same time, physical violence, psychological aggression, child labour and nutritional deficiencies remain relevant risks; a 2016 survey recorded physical violence among 64.1 percent and sexual violence among 12.8 percent of the young people surveyed. Adolescents and young people can use youth-friendly health services (Youth-Friendly Health Services), abbreviated YFHS. These services address, among other things, sexually transmitted infections, sexual and reproductive health, adjustment problems and psychological distress. Access is provided through selected service points, health centres, youth centres and Health Information and Service Centres. Services are offered during regular working hours or at suitable additional times. The education and skills development ministry (Ministry of Education and Skills Development), also known as MoESD, is additionally responsible for career guidance, youth coordination, and technical and vocational education and training (TVET). The transition from school to vocational education or work can take place through technical training, skills programmes, entrepreneurship, overseas employment and employment placement. The industry, commerce and employment ministry (Ministry of Industry, Commerce and Employment), or MoICE, and its responsible departments use, among other tools, Bhutan's labour market information system (Bhutan Labour Market Information System), abbreviated BLMIS, the employment registration system (Employment Registration System), abbreviated ERS, and the three-step Prepare, Promote and Place model (3P). In the 2024–25 financial year, 8,274 placements were recorded, including 1,334 overseas employment placements. Youth unemployment was 17.2 percent. Participation in a programme depends on its specific requirements and does not guarantee a job. For marriages, the legal minimum age is 18 for both sexes. The court procedure for a marriage certificate (Marriage Certificate) requires documents and witnesses; child marriages are legally excluded. For the adoption of a Bhutanese child under 18, the process runs from an application to NCWC through suitability and document checks, preparatory counselling and a court hearing to an Adoption Order (Adoption Order). Families provide much of the practical support in old age, illness and care, but this support does not replace a uniform state care benefit. The National Elderly Care Programme combines health promotion, prevention, treatment, rehabilitation and referrals. Integrated community-based assessments can take place at home, where a person lives, in a health facility or in the community. Families may receive some support to develop care skills. In the 2017 census, 43,064 people were at least 65 years old, representing 5.9 percent of the population. 80 percent had at least one chronic condition, and 50 percent had at least two. Older women and people with disabilities may be particularly vulnerable; their autonomy, dignity and access to health and social care remain central requirements. For people needing care, health, rehabilitation, assessment and referral services are available. Practical long-term care, however, is provided mainly by families, communities and primary healthcare. Bhutan has no nationally uniform, documented care-grade, cash-benefit or universal residential long-term-care system. Costs and access therefore depend on the case, programme, institution and available documents. The 2019 disability policy and Bhutan's ratification of the Convention on the Rights of Persons with Disabilities, effective since 13 March 2024, strengthen equal treatment, accessibility and participation; inclusive education and the transition into work still require concrete support. Other transitions include birth registration and access to children's rights, the move from early childhood care into school, entering education or work, parenthood, ageing and care. For children in difficult circumstances or in conflict with the law, NCWC, child welfare services, probation services and courts work together on protection, diversion and restorative measures. In cases of violence or domestic violence, NCWC, the police and courts provide protection, legal advice, accommodation and case management. For acute suicide risk, the Ministry of Health gives 112; Bhutan's national police force (Royal Bhutan Police) can be reached at 113 according to an official emergency notice. Crisis cases require immediate referral and, depending on need, safe accommodation and continuing care.
