The formal health system in Haiti is led by the Ministère de la Santé Publique et de la Population (MSPP), with departmental health directorates and Unités d'Arrondissement de Santé coordinating services. It has three broad levels. Centres de Santé (CS), with or without beds, and Centres Communautaires de Santé (CCS) commonly provide primary care and prevention. Hôpitaux Communautaires de Référence (HCR) and Hôpitaux Départementaux (HD) provide secondary and referral care. Hôpitaux Universitaires (HU) and other specialized hospitals provide tertiary care. Haiti's 2023 national inventory counted 1,061 health establishments: CS accounted for 50.61%, dispensaries and CCS for 36.76%, and hospitals for 12.6%. The baseline infrastructure data are partly based on the 2017–2018 EPSSS survey. Before illness becomes urgent, services can include routine and catch-up vaccination, antenatal and postnatal care, family planning and contraception, nutrition screening, health education, and prevention, testing or surveillance for tuberculosis, HIV, malaria and cholera. Agents de Santé Communautaire Polyvalents (ASCP) support communities through home visits, meetings, mobile clinics and vaccine points. Local leaders and community organizations may help connect people with available services. Mental-health care, reproductive and maternal care, child health and nutrition programs may be provided by public facilities, private providers, NGOs or supported programs, depending on the area and current capacity. Treatment may include outpatient consultations, maternal and newborn care, childhood illness services, infectious-disease programs, hypertension and diabetes care, emergency and trauma treatment, surgery, obstetrics, inpatient care, mental-health treatment and psychosocial support. Named formal facilities include HUEH, Hôpital Universitaire La Paix in Delmas, Hôpital Maternité Isaïe Jeanty & Léon Audain, Hôpital Psychiatrique Mars & Kline, Hôpital Sanatorium de Port-au-Prince, Hôpital Universitaire Justinien du Cap-Haïtien and Hôpital Saint Michel de Jacmel. Their actual availability can change, so local confirmation is needed before travel when possible. For an emergency, people generally seek the nearest functioning CS, CCS, clinic or mobile clinic, which may refer them to an HCR, HD or university or specialist hospital. ASCPs and other community workers can identify urgent needs, provide basic support within their role, arrange referral and help with follow-up. A guaranteed nationwide emergency telephone number or ambulance service has not been verified. Security, road conditions, fuel, transport, hospital capacity and referral communications can all change the time needed to reach treatment. Traditional providers, including Matrones, Houngans, Medsen fey and Rebouteux, form part of Haiti's documented care landscape. Their role, capacity and legal force vary, and nationwide accreditation or coverage has not been verified. They should not replace formal maternity or emergency care when that care is needed. After treatment, follow-up may include discharge instructions, counter-referral to a lower-level facility, chronic-disease monitoring, TB or HIV treatment follow-up, postnatal and newborn care, nutrition recovery, mental-health rehabilitation or psychosocial support. ASCP home visits and community accompaniment can help find people who miss follow-up, but insecurity, distance, shortages and displacement frequently interrupt continuity. No national insurance membership requirement or uniform gatekeeping rule has been evidenced. Access may be through public, private, nonprofit, mixed, NGO or program-supported facilities, while eligibility and availability differ by service and provider. Out-of-pocket spending represented 43.53% of total health expenditure in 2021. Fees, exemptions and free or subsidized services vary by facility, program and donor, and no national fixed waiting time has been established. MSPP nursing norms recognize access to health and social services, patient choice and decision-making, privacy, respect for values and beliefs, and informed consent, but implementation and enforcement are uneven. HIV testing policy requires consent except for blood-donation screening. Patients are generally expected to provide relevant medical history, record or understand consent where required, follow clinical or referral instructions and return for follow-up when feasible. Maternal and reproductive services remain a major access area. As of 31 December 2025, UNFPA-supported integrated services operated in 32 facilities, supplies had been distributed across all 10 departments, and 417 professionals, including midwives, had received training in emergency obstetric and gender-based-violence care. WHO reported in 2026 that about 4.9 million people in Haiti needed health assistance and that only 26% of 268 inpatient facilities were fully operational nationwide; in Port-au-Prince, 37 of 100 facilities were reported as functional. These figures explain why availability, safety and continuity can differ sharply between departments and over time.
