Health in Kyrgyzstan

Kyrgyzstan has an established but fragmented health system. The Ministry of Health sets policy and regulates services, while the Mandatory Health Insurance Fund (ФОМС) manages insurance status, benefit channels and medicine reimbursement. Public and contracted private providers deliver care under the applicable package and referral rules. Primary care is usually the first access point. A family doctor group (ГСВ), Family Medicine Centre (ЦСМ) or General Practice Centre (ЦОВП) can provide examinations, basic treatment, prevention and referrals. In remote villages, a feldsher-midwife post (ФАП) may provide the nearest basic service. Planned specialist care, diagnostics and hospital admission commonly follow a pathway from the family doctor to a specialist or diagnostic service and then, when needed, to a hospital. Direct specialist access exists, but it does not consistently replace primary-care coordination. The State-Guaranteed Benefits Package (ПГГ) includes basic primary care, ambulatory emergency care, selected outpatient specialist services and diagnostics, vaccinations, inpatient treatment under eligibility and referral rules, and medicines for selected conditions. Planned hospital admission normally requires a referral from a ЦСМ, ЦСОВП or specialist together with the required diagnostic information. Without a referral, the patient may face the maximum applicable co-payment. Emergency hospital treatment can later move into a planned or outpatient pathway with different payment rules. Kyrgyz citizens registered with a primary-care provider receive the basic package. Mandatory health insurance (ОМС) status can extend access to inpatient treatment, diagnostics, rehabilitation and medicine reimbursement. Children, pensioners, social-benefit recipients, students, registered unemployed people, military personnel and other statutory groups may qualify for insurance. An uninsured person can apply for an ОМС policy; its terms and cost are reviewed annually. Workers from the Eurasian Economic Union (EAEU) and their families have a specific access pathway. The pathway for non-EAEU foreigners is not fully consolidated in the reviewed official material, so providers or ФОМС should confirm the applicable conditions rather than relying on a general assumption. Emergency ambulance service at 103 operates 24 hours a day. It responds regardless of insurance status or citizenship and remains free until the immediate life threat has been resolved and the patient has been stabilized. This rule does not make every later hospital, diagnostic or medicine cost free. Highly specialized care is concentrated in Bishkek, so travel, workforce shortages and supply gaps can make access more difficult from rural and mountain areas. No nationwide waiting-time standard was identified; actual timing depends on the facility, referral, region and available staff or equipment. Prevention includes national vaccination, maternal and child care, and screening or treatment pathways for noncommunicable diseases, tuberculosis and HIV. A 2025 catch-up campaign addressed missed doses for DPT-3, measles, rubella, pertussis, polio and diphtheria. MICS 2023 recorded basic vaccination coverage of 71.6% among children aged 12 to 23 months and about four in five children aged 24 to 35 months. Digital vaccination records through iЭмдөө, the Tunduk platform and expanding electronic health records support documentation, although availability differs by service and location. Follow-up usually returns to the family doctor for chronic-disease monitoring and continued treatment. Electronic subsidized prescriptions can be filled through contracted pharmacies. The package provides free outpatient medicines for diabetes, hemophilia and tuberculosis, and subsidized medicines for paranoid schizophrenia, affective disorders, epilepsy, asthma and terminal oncology. Hypertension medicines receive a 50% subsidy of the basic price. Insured ОМС patients may receive additional medicine reimbursement, often up to 50%. Rehabilitation and physiotherapy are free for insured patients and generally paid for others, while palliative care belongs to the state-guarantee framework. Patients have a constitutional right to health, medical insurance and a public health network within statutory guarantees. Providers may require an identity document, proof of benefits, the 14-digit personal identification number (ПИН) or evidence of ОМС status. Co-payments remain common for outpatient and inpatient care, and the World Health Organization reported that out-of-pocket spending represented 40.7% of health spending in 2021. Reimbursement lists generally cover about 50% where a subsidy applies, but actual payment depends on the service, medicine, eligibility and provider. There is no single current nationwide public directory that reliably combines facility opening hours, fees, medicine stock and waiting times, so local confirmation is often necessary.
